As the 2026 midterm elections approach, there are several overlapping forces that create instability for healthcare policies, according to Mike Ciarametaro, managing director of policy at Avalere Health, who recently sat down with Managed Healthcare Executive to discuss some key points from his talk.

In this conversation, he highlights the growing confusion around Most Favored Nation (MFN), particularly the GENEROUS model, which is the Medicaid MFN model that may be delayed because the language about which companies are included is unclear.

“Separately, the administration has specified that they want to codify MFN into law,” Ciarametaro said in the interview. “With midterm elections coming up, Democrats aren't going to give the Republicans a win, so the only way to do that is through the reconciliation process, and that is unlikely to happen.”

Ciaramatero presented today at Asembia’s 2026 Specialty Pharmacy Summit, being held in Las Vegas from April 26 to April 30.

He spoke during a session called, ‘Healthcare and Drug Pricing at a Crossroads: Policy Directions Ahead of the Midterms.’
