Claim Denial Rates Increase for Third Consecutive Year, Survey Shows

Data quality issues and increased rates of claim denials provide opportunities for artificial intelligence to make improvements, according to a recent survey by Experian Health.

Approximately 41% of providers face claim denial rates of at least 10%, according to the results of the 3rd annual 2025 State of Claims survey from Experian Health.

This year’s results contribute to an ongoing trend of denials, and although 67% of respondents believe artificial intelligence could improve the claims process, only 14% of providers are currently using it.

“The sustained increase in claim denials, coupled with persistent data quality issues, paints a clear picture that providers need to lean into technology and AI’s potential to address these challenges,” Clarissa Riggins, CPO at Experian Health, said in the news release. “Our survey underscores that AI is no longer just a theoretical solution, but it’s a vital tool that can break the pervasive cycle of denials, delays, and data errors.”

Results are based on an online survey of 250 healthcare professionals who work with finance, billing or claims management. Answers were gathered between June 23 and July 3, 2025.
