Dr. Crowson is betting on the role of the medical home as a concept for transforming primary care and integrating it with DM. "The medical home's care systems can be designed to integrate DM functions," he says. For example, patient registries can be developed and maintained by the medical home to help identify patients who are at risk for certain conditions.
