In the March 16 Morbidity and Mortality Weekly Report, a summary of cases reported to the CDC in 1999 found that Connecticut, Rhode Island, New York and Pennsylvania had the highest incidence per 100,000 population; children and adults aged 45 to 59 comprised the majority of cases.
