As a combination of provider relations, care and disease management, pharmacy benefits management and specialty pharmacy, Comprehensive Health Solutions Business Unit provides an industry best practice in population-based healthcare management through its 360° Health programs. "The advantage of this integration is the ability to link multiple databases, utilize data analytics and apply them to outcomes through a set of algorithms," Dijuana Lewis says. "Our primary goal is to help members stay well and avoid chronic health conditions in the future rather than just helping people when they are sick."
Members with asthma reported increased adherence to their medication by about 1% to 5% and compliance with their asthma action plan of 3 to 9% over two years. Members with coronary artery disease (CAD) showed increased compliance of their disease-related medications by 1% to 4% and improved their blood pressure control by 3% to 6%. As of January 2008, 360° Health DM programs contributed to a 15% decrease in emergency room visits, 5% in inpatient admissions and a 7% drop in inpatient days.
